68M WITH PYREXIA and COPD

This is an online elog book to discuss our patient’s health data after taking his consent.This also reflects my patient centered online learning portfolio. Here we discuss our individual patient’s problems through series of inputs from available global online community of experts with an aim to solve those patient’s clinical problems with collective current based evidence based inputs. CHIEF COMPLAINTS- A 68yr old male patient who is farmer by occupation resident of Tangedpalli came with the chief complaints of- -fever since 2 days -difficulty in breathing since 1 day -facial puffiness since 2 days  HISTORY OF PRESENT ILLNESS- Patient was apparently asymptomatic 2 days back.Then he developed fever which was low grade,intermittent in nature ,relieved on medications associated with chills and rigor,  facial puffiness,generalised body pains.No diurnal variations. H/o shortness of breath since 1 day which was sudden in onset,

A 52 yr old male patient farmer by occupation came to the OPD with chief complaints of fever since 10 days . HISTORY OF PRESENT ILLNESS- Patient was apparently asymptomatic 10 days back.Then he developed fever which was high grade,sudden in onset, intermittent in nature aggrevated during night and on exertion. It is associated with body pains(aggrevated on exertion),chills and rigor.It is not associated with cough,cold,vomitings,headache. Patient has complaints of Shortness of breath on exertion since 10 days.No PND,Orthopnea. PAST HI

A 20 yr old female who is a student and resident of Nalgonda came to the OPD with chief complaints of fever since 1 day , loose stools since 1 day and pain abdomen since 1 day HISTORY OF PRESENT ILLNESS- Patient was apparently asymptomatic 2daysback.Then she developed fever which was sudden in onset,high grade,associated with chills and rigor,relieved on medications. No diurnal variation is seen.Not associated with cough and cold. C/o loose stools which was watery in consistency, non blood stained, non-foul smelling ,4-6 episodes per day.It

A 50 yr old female patient came to the OPD with the chief complaints of fever and cough since 6 days. HISTORY OF PRESENTING ILLNESS- Patient was apparently asymptomatic 6 days back. Then she developed fever which was sudden in onset, intermittent in nature and high grade. It was associated with headache, chest pain.It was not associated with nausea,vomiting and no diurnal variations. She also developed cough which was associated with sputum. The sputum was white in colour,non foul smelling and non blood stained. PAST HISTORY- H/o HTN since 3 years and DM since 3 months. N/K/C/O asthma,Epilepsy ,TB PERSONAL HISTORY- DIET-Mixed Sleep-adequate Appetite-normal Bowel and bladder movements-regular No addictions FAMILY HISTORY-Not significant GENERAL EXAM

A 48 year old male patient came with the chief complaints of cold, cough and fever since 10 days. HISTORY OF PRESENTING ILLNESS: The patient was apparently asymptomatic 10 days back, then he developed        Fever : sudden in onset                    Intermittent in nature                    High grade Not associated with nausea, vomiting, chills, rigors or headache. The cough was associated with sputum which was non blood stained.   PAST HISTORY: N/K/C/O DM , TB, HTN,  epilepsy, Asthma and thyroid. No history of similar complaints in the past. PERSONAL HISTORY: Appetite - decreased Sleep - adequate  Diet - mixed Bowel and bladder movements - regular No addictions FAMILY HISTORY: Not significant  GENERAL EXAMINATION: Patient was C/C/C and slightly malnourished  No pall

This is an online elog book to discuss our patient’s health data after taking her consent.This also reflects my patient centered online learning portfolio. * HISTORY OF PRESENTING ILLNESSES: * The patient was apparently asymptomatic 1 yr back then developed back pain which was severe, gradually progressive and non radiating. The pain was aggravated on standing and relieved on rest and medication. The pain was more during night and was associated with difficulty in breathing. * PAST HISTORY: * H/o trauma N/K/C/O DM, HTN, Asthma or epilepsy PERSONAL HISTORY- Appetite-normal sleep-adequate Fdiet-mixed Bowel and bladder movements-regular No addictions  * FAMILY HISTORY: * not significant * GENERAL EXAMINATION: * Patient was C/C/C and moderately nourished No pallor No icterus No cyanosis No clubbing No lymphadenopathy No pedal oedema

 14 October, 2023 12yr old male patient with chief complaints of dry cough. Hi ,I am Ananya Nayak, 5th sem medical student. This is an online elog book to discuss our patient’s health data after taking his consent.This also reflects my patient centered online learning portfolio.  A 12 yr old male came to the OPD with chief complaints of dry cough since 15 days. HISTORY OF PRESENTING ILLNESS- The patient was apparently asymptomatic 15 days back. Then he developed severe dry cough on exposure to dust which was associated with shortness of breath.The cough aggrevated in the night time. It was not associated with fever,vomiting and headache. PAST HISTORY- He had similar episodes since 2-3yrs due to unknown allergy. N/K/C/O DM,TB,HTN,EPILEPSY.  PERSONAL HISTORY- Appetite-Normal Sleep-adequate  Bowel and bladder movements-Regular Diet-mixed  FAMILY HISTORY-irrelevant GENERAL EXAMINATION- Patient was C/C/C and moderately built. No pallor  No icterus  No cyanosis  No lymphadenopathy  No pedal
