28 October 2023 Hi ,I am Ananya Nayak, 6th sem medical student. This is an online elog book to discuss our patient’s health data after taking his consent.This also reflects my patient centered online learning portfolio. Here we discuss our individual patient’s problems through series of inputs from available global online community of experts with an aim to solve those patient’s clinical problems with collective current based evidence based inputs. CHIEF COMPLAINTS- A 68yr old male patient who is farmer by occupation resident of Tangedpalli came with the chief complaints of- -fever since 2 days -difficulty in breathing since 1 day -facial puffiness since 2 days HISTORY OF PRESENT ILLNESS- Patient was apparently asymptomatic 2 days back.Then he developed fever which was low grade,intermittent in nature ,relieved on medications associated with chills and rigor, facial puffiness,generalised body pains.No diurnal variations. H/o shortness of breath since 1 day which was sudden in onset,
